Sliding Fee Option

We are committed to making care accessible. Clients who are experiencing financial hardship may request a reduced self-pay fee based on our Sliding Fee Scale, which considers household income and family size. Please note sliding scale patient slots are limited.

To request a sliding fee adjustment, clients must:

• Request a Sliding Fee on this Form
• Provide verification of household income, such as recent pay stubs or tax returns.
• Request Sliding Fee before or during the intake process.
